Willow House Community Support Ltd delivers a structured 6-week Community Stabilisation & Participation Program designed to support individuals experiencing instability, financial hardship, social isolation, or justice system involvement.

Program Structure

Each participant engages in a structured six-week process:


Week 1 – Intake & Goal Setting

  • Structured intake discussion

  • Identification of stabilisation goals

  • Risk and protective factor review

  • Referral mapping (if required)


Weeks 2–5 – Structured Engagement

  • Weekly psychosocial support sessions

  • Supervised community participation activities

  • Progress review and skill development

  • Referral follow-up and coordination


Week 6 – Exit Planning

  • Review of progress achieved

  • Ongoing support recommendations

  • Community connection planning
